Constipation is a heterogeneous disorder with a wide spectrum of symptoms and complaints. Constipation is a highly prevalent and bothersome disorder that negatively affects patients' social and professional lives and places a great economic burden on patients. Osmotic laxatives are widely prescribed in the management of constipation due to their superior efficacy, tolerability and safety in pediatric patients, during pregnancy and elderly population.

Psyllium or Ispaghula is the common name used for several members of the plant genus Plantago whose seeds are used commercially for the production of mucilage. Laxatives that act by absorbing water, increasing the bulk and softening the consistency of the stools. They consist either of plant fibre (e.g. bran, ispaghula husk, sterculia) or of a synthetic agent (e.g. methylcellulose).

Bulking agents are usually effective 12–24 hours after being taken, but it can take some days for their full effect to develop. They are useful for encouraging bowel movements in people with a colostomy, ileostomy, haemorrhoids, or anal fissure and for treating chronic diarrhoea in people with irritable bowel syndrome or diverticular disease, when their ability to absorb water in the gut is exploited to decrease the liquidity of the stools.Preparations containing ispaghula should always be taken with plenty of fluid and should not be taken immediately before going to bed.

Lactitol is a synthetic disaccharide osmotic laxatives. With its more pleasant taste, lactitol is better tolerated and more palatable. This article reviews the pharmacological properties of lactitol, as well as clinical trials evaluating its role in constipation. Data from comparative study with Lactulose shows that Lactitol offers superior efficacy, better tolerability and palatability. Cathartic effect of Lactitol is more predictable. Lactitol appears to be the ideal successor to lactulose for the treatment of Constipation. Lactitol is a sugar alcohol used as a replacement bulk sweetener for low calorie foods with approximately 40% of the sweetness of sugar. It is also used medically as a laxative.
